Would like to be corrected about this, but after a bit of research it appears that launching COD2 or COD4 and joining a server from a web link will never be possible as the game does not install a URL handler into your machines registry.
The most popular Third Party Launcher appears to be Xfire, so here are the links for it. You can download XFire from here
(XFire must be running and have correctly detected the game on your system for the following links to work)
